Be Prepared For Your Child’s First Appointment

Print out and complete these forms before your child’s first visit to Capital City Children & Adolescent Clinic, PLLC. If you have any questions on how to fill out certain fields, call our office for assistance.

Medical & vaccine records need to be sent to our office at least one week prior to your child's appointment. Where do they email them to?
Bring insurance card to each visit.
